Fuel card agreement

1. Purpose

The Company issues fuel cards to drivers strictly for authorized company business. This Agreement establishes the Driver’s responsibilities regarding the use, protection, and security of Company-issued fuel cards.

2. Authorized Use Only

Fuel cards may be used only for:

Any personal use, third-party use, or use outside assigned operations is strictly prohibited.

3. Fuel Card Security & Mobile Application Requirement

The Driver agrees to: Install and use the Company-approved fuel card mobile application Unlock the fuel card only at the time of fueling Acknowledge that card unlock access is time-limited (approximately 30 minutes) Never share fuel card details, PINs, usernames, passwords, or unlock access Keep the fuel card and mobile device secure at all times The Driver understands that failure to follow these security procedures increases the risk of fraud and unauthorized use.

4. Prohibited Conduct

The following actions are strictly prohibited: Sharing the fuel card or card information with any other person Allowing another individual to use the fuel card Fueling non-company vehicles Attempting to bypass or manipulate card security features Using the card without proper authorization

5. Responsibility for Unauthorized Use

The Driver agrees that any unauthorized use of the fuel card resulting from: Sharing the card or credentials Negligence or failure to follow Company procedures Failure to secure the card or mobile device may result in: Full financial responsibility for unauthorized charges Disciplinary action, up to and including termination Deduction from final pay where permitted by law Civil recovery actions if necessary

6. Lost, Stolen, or Compromised Card

The Driver must immediately notify the Company if: The fuel card is lost or stolen The mobile device used for card access is lost or compromised Any suspicious or unauthorized activity is detected Failure to report promptly may result in liability for resulting losses.

7. Training and Acknowledgment

By signing this Agreement, the Driver confirms that they: Have received instruction on proper fuel card usage Understand Company fuel card policies Agree to comply with all security requirements Understand the consequences of misuse

8. Governing Law

This Agreement shall be governed by and interpreted in accordance with the laws of the state in which the Company is registered.
